Registered Practical Nurse - Long Term Care (Temporary Part-Time)
Responsible for providing care toclients as well as supervision and direction to staff including and notlimited to PSWs, support services, etc. on a designated unit. The RPN supports the mission, vision andvalues of SJHCG, as well as working towards established goals andobjectives. The RPN is alsoresponsible for following all policies, processes and procedures of SJHCG.

Key Accountabilities

Performs allduties in accordance with legislation, the Standards of Nursing Practice ofthe College of Nurses of Ontario, the College of Nurses Guidelines forProfessional Behaviour, the Mission Statement, and policies and procedures of St. Joseph’s Health Centre Guelph.

The Teaching-Coaching-Counselling Function

Providinginterpretation of the individual’s condition and giving rationale forprocedures

The Diagnostic and Monitoring Function

Performingnursing assessment of clients with medical and psychiatric needs, referring/collaboratingwith the Registered Nurse in performing nursing assessment of older adultswith complex medical and psychiatric needs

Detectingand documenting significant changes in a clients’ condition

Anticipatingdeterioration prior to explicit confirming diagnostic signs, and anticipatingchallenges and problem solving with others

Understandingthe particular demands of an illness and anticipating care needs

Assessingthe individual’s potential for wellness and for responding to varioustreatment strategies





Effective Management of Rapidly Changing Situations

Managingextreme life-threatening emergencies before and after the arrival of theresuscitation team

Identifyingand managing a client in crisis

Administering and Monitoring Therapeutic Interventions and Regimes

Performingand monitoring vascular access (VA) therapy (hypodermoclysis)

Promotingsafety, comfort, hygiene, Promoting respiration / oxygenation

Monitoringvital signs, weight change

Administeringmedications

Promotingfluid balance, nutrition and digestion, elimination, posture, mobility andambulation

Preventingand controlling infection

Promotingskin integrity, neurological status, balance between rest and activity, psychosocial/mental/emotionalfunction

Obtaining,preparing, and preserving specimens

Assistingwith diagnostic procedures

Planning andimplementing nursing interventions, then evaluating the outcomes of nursinginterventions for older adults with medical and psychiatric needs

Supportingclient/family access to health care and community resources

Establishingand implementing education plans with and for clients and families

Establishing and Maintaining Healthy InterpersonalRelationships

Demonstratingenthusiasm for working with older adults and their families

Developingand preserving therapeutic nurse/client relationships, developing andpreserving collaborative relationships among members of the interdisciplinaryteam

Developingand preserving open communication between members of the health care team andthe client’s social support network

Developingand preserving ongoing team-oriented relationships with community agenciesand personnel

Developingand preserving collaborative relationship with the Director/ Nurse Manager.

Providingsupportive direction and leadership for PSWs and clerks where appropriate

Collaborates with the Nurse Practitioner/physicianin accordance with their respective roles.

Qualifications & Experience

Registered Practical Nurse– Registered with the College of Nurses of Ontario, in good standing

Minimum one (1) yearexperience with clients in long-term care home setting preferred.

Superb organizationalplanning, time management and multi-tasking skills

Demonstrated written andverbal communication skills, and excellent interpersonal and leadershipskills

Exceptional client serviceskills

Basic computer skills arerequired
