We're seeking a Home Support Supervisor to oversee community-based nursing care in Peel, North York, Scarborough, York, and Toronto. As a Registered Practical Nurse, you'll lead and develop your PSW team.

Your primary focus will be on patient assessments and setting care goals while ensuring your team is well-trained and supported. You will manage staff performance and maintain awareness of key indicators that affect service quality and patient satisfaction.

Key Responsibilities:
• Conduct home visits to assess patient needs
• Formulate and implement care plans with the team
• Manage staff through performance evaluations
• Record and analyze feedback for service improvement
• Address patient concerns efficiently

Requirements:
• Must be registered with CNO in valuable standing
• At least three years of direct supervisory experience
• Valid driver's license and access to transportation
• Strong solution-oriented interpersonal skills
• Proficient in English communication, additional languages are a plus
