Job Description 17 full-time positions available Wage: $34.45 per hour Travel Locations: Oshawa, Whitby, Ajax, Pickering, Bowmanville, Uxbridge Shift Options: 0800-1600, 0900-1700 , 1200-2000, 1400-2200 and 2300-0700 In this community nursing role, as a Registered Nurse (RN) you will have scheduled visits to care for patients in various settings. You will work with patients of all ages with differing complexity of care needs and play a key role in helping patients remain comfortable in their homes for as long as possible.

What you’ll do: Using appropriate assessment tools/techniques to assess patients’ individualized needs
Executing and continuously monitor care plans
Engaging patients using a collaborative approach by building rapport and trust
Maintaining and update professional patient records related to patient encounters and plans
Assess patients to identify appropriate nursing interventions
Collaborate with members of an interdisciplinary health team to plan, implement, coordinate and evaluate patient care in consultation with patients and their families
Administer medications and treatments as prescribed by a physician or according to established policies and protocols
Monitor, assess, address, document and report symptoms and changes in patients' conditions
Operate or monitor medical apparatus or equipment




Assist in surgery and other medical procedures
May supervise licensed practical nurses and other nursing staff
May develop and implement discharge planning process on admission of patients
May teach and counsel patients and their families on health-related issues in collaboration with other health care providers
Job Requirements: Nursing degree or required education mandatory for CNO licensure
Registration with the CNO as a Registered Nurse in good standing
Clear Vulnerable Sector Screen
Negative TB Test
Fluent proficiency in written and verbal English
Valid driver’s license (Class G or G2), access to a dedicated vehicle is required and valid car insurance
Current and valid CPR certificate
Experience in delivering community care is an asset
Reliability and flexibility
Willingness to travel to and from patient residences within a designated area
Passion for making a difference in your community
Required on-call coverage, including after hours to ensure patient safety and to meet patient care needs
