Key Responsibilities:
• Facilitate resident care conferences and schedule meetings
• Ensure care plans are accurate and up-to-date
• Implement care services and evaluate their effectiveness




• Teach and direct unregulated care providers
• Respond to medical emergencies and provide first aid

Requirements:
• Active LPN registration in the applicable province
• Prior experience in gerontology and rehabilitation recommended
• Solid skills in care coordination and communication
• Basic Cardiac Life Support certification required
• Ability to perform a comprehensive Vulnerable Sector Check
