You will collaborate with onsite healthcare teams to ensure optimal resident care while conducting clinical pharmacy activities in accordance with established standards. This role focuses on education and relationship-building within the community.
Key Responsibilities:
• Liaise between long term care homes and pharmacy teams • Conduct medication reviews in serviced communities • Provide education on health topics including MedsCheck • Participate in medication audits and clinical meetings • Facilitate ongoing collaboration with healthcare professionals
Requirements: • Licensed pharmacist in good standing with Ontario College • Proven experience fostering healthcare partnerships • Robust organization and time management skills • Excellent interpersonal and communication abilities • Familiarity with provincial LTC regulations
