The Social Worker reports to the Director of Care, contributing significantly to resident and family support by providing counseling, education, and community resources. You'll complete comprehensive psychosocial assessments, engage with families, and establish therapeutic relationships. Your impactful work aids in addressing the psychological, social, and spiritual needs of residents.
Key Responsibilities:
• Conduct comprehensive psychosocial assessments for newly admitted residents • Establish rapport and therapeutic relationships with residents and families • Facilitate interviews to gather vital personal information • Provide consultation, adjustment support, and counseling as needed • Collaborate within a multi-disciplinary care team for holistic service delivery
Requirements: • Registered Social Worker under the Health Professions Act • Bachelor's degree from a recognized school of social work • Experience working with geriatric populations • Effective teamwork skills in a multi-disciplinary setting • Current Vulnerable Sector Check or willingness to obtain
