Join The Health Centre as a Primary Care Connection Lead in Waterloo Region and enhance health service access for the community. This full-time role focuses on client intake and coordination.

You will be responsible for directing the intake processes and ensuring timely, effective triage for clients requiring health services. As a leader, you will work collaboratively with healthcare teams and partners to identify gaps in service and drive strategic improvements.

Key Responsibilities: • Oversee and coordinate intake and referral workflows • Conduct assessments for client eligibility and service level • Maintain accurate health records in compliance with policies • Engage with external healthcare networks and community organizations • Report on key performance indicators for intake success

Requirements: • Must hold a registration as a Nurse in Ontario • At least two years’ experience in relevant health settings • Experience in triage and decision-making processes • Familiarity with community health resources • Robust interpersonal skills and team collaboration ability
