CARE AND BE CARED FOR – THIS IS YOUR HOME Reporting to the Manager, Placement Services, the Facilities Coordinator ensures that the processing of placements for long-term care facilities meets the Ontario Health atHome standards.
What will you do?
Act as a liaison to staff of hospitals and long-term care facilities process applications at assigned hospitals by providing information to patients and their families to support their decision-making about placement in long-term care facilities
Prioritize and authorize admissions to long-term care facilities according to legislated criteria while managing and adhering to the facility waiting list
Effective and frequent communications with long-term care facilities, hospitals, Coordinators, patients, families, caregivers, co-workers and community agencies
What must you have?
A university degree with a major in Social Work, Psychology, Nursing, Occupational Therapy, Physiotherapy, or Gerontology or two (2) years of recognized work experience in a placement department exposed to admissions to long-term care.
Knowledge of relevant legislation, long-term care facilities and community resources.
Working knowledge of resources to support patients with responsive behaviours
Good understanding of Ontario’s health care system, specifically the Behaviour Supports Ontario, and of the different service providers across the care continuum
Demonstrate excellent interpersonal, communication and customer service skills with a superior capability to interact sensitively with service partners from hospitals and long-term care facilities, and patients and their families
Ability to work independently and co-operatively in a busy multi-disciplinary situation
Experience working with physically and/or cognitively impaired elderly and disabled adults and their families
Demonstrated computer literacy in a Windows environment, particularly with MS Word and Excel
Cultural sensitivity and respect for the diverse population in Toronto is required
What would give you the edge?
Experience working in a long-term care environment an asset
Fluency in languages other than English is an asset
Placement experience is an asset.

Salary: $25.4810 to $30.3613 Per Hour
