OVERVIEW
Reportingto the Southern Care Program Supervisor, the South Care Case Manager will beresponsible for providing support to the South Care homes. The Southern CareCase Manager will work within a team that serves to provide comprehensive casemanagement services within a holistic circle of care.
KEYRESPONSIBILITIES
- Facilitate pre-service orientation for new CareProviders.
- Establish a positive and supportive relationshipwith Care Providers, youth in the homes, and the support team through being aregular visitor to the home and maintaining ongoing communication by phone andvia email.
- Conduct intake care planning meetings with guardianagency staff and Care Providers and conduct regularly scheduled care planningupdate meetings.
- Based on your ongoing awareness of and connectionto the homes, complete ongoing care home documentation and assess and completerequired licensing documentation:
- HomeStudies and Foster Home License Applications, as needed.
- AnnualReviews
- Addendums,as needed.
- FosterCare Licensing Closures, as needed.
- Maintain an accurate understanding of the servicesprovided in each home and effectively communicate with the Program Supervisorand guardian agency.
- Collaborate with guardian worker in delivering careplans for each child and/or family, while remaining focused on child’swholistic needs.
- Collect and review monthly reports and incidentreports for each home, identify challenges and strategize with the home andcare teams to implement solutions.
- Liaise with Care Providers to ensure information iseffectively communicated.
- Communicate to Supervisor the observed trainingneeds for House Parents, Support & Respite Workers, and find those trainingopportunities.
- Contribute to organization committees based on yourpersonal and professional experience and goals.
- Regular travel is required for this position(Winnipeg and surrounding areas) – travel and accommodation provided.
- Ability to lift and carryitems to a maximum of 50lbs
QUALIFICATIONS
- BSW degree or equivalent(similar education and experience working in the field will be considered)
- Knowledge in the areas ofthe impacts of colonization and intergenerational trauma
- Excellent conflictresolution skills
- Knowledge of the Child andFamily Services (CFS) sector and legislation
- Good understanding offoster care standards and related licensing paperwork
- Demonstrate knowledgeand/or understanding of Indigenous perspectives, First Nations culture andtraditional philosophies.
- Experience developing andsupporting a strong, respectful team-based approach to service delivery.
- Experience working withfamilies and navigating complex situations.
- Must have a transparent ChildAbuse & Adult Abuse Registry Check, Criminal Record check (withVulnerable Sector).
- Strong proficiency inMicrosoft Office, word processing (MS Word), Spreadsheets (Excel), E-mail(Outlook) and Internet.
- Excellent communication,organization, problem solving and team skills.
- Ability to speakAnishinaabe is an asset.
- Must have a valid Manitobadriver’s license and a reliable vehicle.
