Under the supervision of the Sr. Clinical Manager, this RN role emphasizes care planning, clinical education, and member support. You will work closely with high-risk members and guide them to optimal care solutions. Responsibilities include identifying barriers to care, conducting outreach, and leveraging data analytics for informed decision-making.

Key Responsibilities:



• Perform clinical triage using evidence-based protocols • Conduct high-risk outreach to engage members proactively • Assist members in scheduling appointments and referrals • Document nursing assessments and care guidance accurately • Serve as a trusted health advocate throughout care journeys
