Be a key player at Scarborough Health Network as a Social Worker on the Interprofessional Primary Care Team. This part-time position involves impactful clinical services and community engagement to address complex health challenges.
You will assess and plan to address the social determinants that affect patient health and recovery while collaborating with a multidisciplinary team. Your role will include providing counseling, facilitating referrals to community services, and enhancing patient independence through meaningful support and communication.
Key Responsibilities:
• Collaborate in the assessment and planning of care • Provide therapeutic counseling and educational services • Document patient interactions and monitor care plans • Facilitate effective communication between care teams • Conduct research to improve quality of care practices
Requirements: • Must hold an MSW and be registered with the Ontario College • Experience in community or primary care is essential • Excellent crisis intervention and assessment abilities • Robust commitment to health equity principles • Able to communicate clearly and effectively
