Monitor the provision of elder care by the nursing team on an assigned unit, acting as the team leader. Assess the elder’s needs in accordance with the RAI-MDS tool and provide a plan of care and interventions based on an evaluation of outcomes.
Duties And Responsibilities
- Administer and document medications and treatments for all elders, and assess effectiveness
- Report elder response to medication and treatments to physician/nurse practitioner
- Transcribe and process all orders received
Communication And Documentation
- Chart care provided, behaviours and incidents
- Respond to elder and family inquiries
- Educate elders, families, and staff
- Complete RAI-MDS assessments, RAPs and care plans according to schedule
- Act as a mentor for students during clinical placement
- Provide guidance and direction to Personal Support Workers, and other staff working on the assigned unit
Elder Care
- Assist as necessary with elder care and transferring
- Ensure elder and family are comfortable during palliative phase
- Monitor levels of liquid oxygen in portable ranks and fill as necessary
General
- Incumbents must keep up-to-date on all relevant legislation, collective bargaining agreements, County Policies and Procedures, etc. ensuring that these are implemented as prescribed
- Incumbents must possess the physical ability to meet the duties and responsibilities of the job description and/or the requirements identified within the position's Physical Demands Analysis
Credentials Required
- Registered Practical Nurse Diploma
- Registration with the College of Nurses of Ontario
