LMC Healthcare is looking for a proactive Diabetes Educator - Registered Dietitian in Brampton, Ontario. This full time, hybrid role focuses on empowering patients with diabetes management skills.
The Diabetes Educator role at LMC Healthcare offers a unique chance to impact the lives of individuals managing diabetes. You will educate and support patients in individual and group settings, while collaborating with the interdisciplinary team on care plans. Staying current with evidence-based practices, you'll also handle administrative tasks, ensuring an organized approach to patient care.
Key Responsibilities:
• Provide diabetes self-management education
• Collaborate on developing individualized care plans




• Complete detailed patient documentation in EMR
• Educate on insulin administration and SMBG
• Perform remote follow-ups and community outreach
Requirements:
• Valid dietitian license with the College of Dietitians of Ontario
• Experience in individual and group counseling
• CDE designation or in progress
• Access to a personal vehicle with a valid license
• Knowledge of motivational interviewing techniques
