Key Responsibilities:



• Perform assessments for new patient services • Create tailored care plans based on assessments • Conduct safety assessments of living conditions • Guide scheduling of ongoing patient care • Maintain thorough and accurate patient records

Requirements: • Active RPN/LPN registration with two years’ experience • Knowledge of applicable healthcare laws • Strong written and verbal communication skills • Demonstrated patient-centered care competency • Access to a personal vehicle required
