Become part of the future of musculoskeletal care! Kingston Health Sciences Centre is recruiting qualified Physiotherapists and Chiropractors registered in Ontario to become contracted APP’s delivering patient assessment, education, self-management advice, and triage for lumbar conditions at their respective practice locations.

The Rapid Access Clinics for Low Back Pain (RAC LBP) have been implemented across the province to help patients access the right treatment faster. RAC LBP stems from the successful pilot program: Inter-professional Spine Assessment and Education Clinics (ISAEC) for low back pain (LBP).

RAC LBP is an innovative, shared-care model funded by the Ministry of Health. Ontario South East’s RAC LBP is hosted by Kingston Health Sciences Centre with APP clinicians contracted across the region. By providing the right care at the right time, RAC-LBP is designed to decrease the prevalence of unmanageable chronic low back pain, reduce unnecessary diagnostic imaging, and reduce unnecessary specialist referrals while expediting surgical consultation and access to specialist care when appropriate. RAC LBP is specifically for patients with: (1) persistent low back pain and/or related symptoms (e.g. radiculopathy, neurogenic claudication), 6 weeks to 12 months post-onset; and (2) disabling episodic low back pain and/or related symptoms of less than 12 months post-recurrence.

As a member of the RAC LBP, the APP will:

perform a thorough history and physical examination

provide a program of care to the patient with an emphasis on education, self-management and health promotion strategies

develop a collaborative care plan with the patient, their primary care provider, and/or other allied health providers and document progress against the care plan

identify signs and symptoms requiring further diagnostic imaging/testing, referral to the practice lead/physician specialist/surgeon, or a change in the plan of care.

act as the primary point of contact for consultation with the patient’s primary care provider

This is an excellent opportunity for advanced professional development in low back pain management. The APP will be connected to the regional Practice Lead and Physician Specialists who will provide clinical guidance and advanced low back pain education. Additionally,



all APPs will be connected through a centralized referral and case management system.

Please note:

Successful candidates will complete both paid practical training (approximately 4-5 days in-person over 4-6 weeks) and theoretical training (online) in low back pain topics. Following the formal training period, participation in clinical education sessions is an expectation of the APP role. Mandatory meetings will be compensated.

Qualifications and Job Requirements:

Minimum 4 years of clinical experience following completion of an accredited physiotherapy or chiropractic program.

Including, at least 1 year of clinical experience in the musculoskeletal-spine area required.

Member in good standing of respective professional/legislativeOntariocollege required.

Minimum $5,000,000 Liability Insurance required.

Member of related national/provincial qualified associationpreferred.

Possession of a laptop/Chromebook, tablet, and printer.

Convenient practice location with internet. Accessible via public transportation preferred.

Key Responsibilities:

Expert Practice

Act as the first and primary point of contact for consultation and assessment of referred patients with low back pain.

Review medical history/health status and conduct a comprehensive physical assessment.

Communication of a diagnosis.

Provide specialized care, education and counseling for patients.

Determine the suitability and eligibility of patients for surgical assessment.

Differentiate between inflammatory and non-inflammatory conditions.

A broad understanding of indications for evidence-based low back treatments.

Identify signs and symptoms of undiagnosed medical conditions requiring a referral for diagnostic imaging/testing and/or referral to a Physician Specialist in accordance with established RAC LBP guidelines.

Initiate communication with primary care providers and Practice Lead to facilitate appropriate surgical and non-surgical referrals based on established RAC LBP assessment and referral guidelines.

Establish, communicate and document clinical impressions/medical diagnoses within defined scope of practice.

Develop plan of care in collaboration with the patient/family and primary care provider.

Promptly document plan of care according to established RAC LBP guidelines.

Communicate internally and externally to facilitate plan of care (i.e. other health care professionals).





Evaluate and revise the plan of care in collaboration with patient/family and primary care provider.

Communicate specific medical diagnoses to primary care provider.

Utilize a patient-centered care approach consistent with patient goals, needs and values.

Teaching and Learning

Participate in RAC sponsored education and training events.

Collaborate with the RAC team to identify learning needs and those of patients and primary care providers.

Provide/receive education to/from RAC team members and primary care providers.

Facilitate knowledge transfer and evidence-based practice.

Organization Responsibilities

Act as an engaged member of the RAC.

Accountable to align performance with objectives of the RAC.

Adhere to the RAC standards, policies and procedures, including centralized referral and clinic management system processes.

Participate in quality and performance management activities.

Participate in policy and procedure development.

Program Evaluation

In collaboration with RAC colleagues, you will cooperate with evaluations of clinical services related to the program.
