Step into the role of Temporary Youth Case Manager at CMHA-WECB, specializing in supportive housing. This role combines housing advocacy and mental health support, targeting youth experiencing homelessness.
Operating under the guidance of the Management team, you will provide comprehensive case management and education services to youth with mental illness. Your work will support clients in securing and maintaining protected housing, while also promoting their overall wellness through skill-building and community reintegration.
Key Responsibilities:
• Engage clients in developing personalized treatment plans • Share education on managing mental health symptoms • Conduct assessments to identify individual needs • Advocate for resources and community support • Facilitate group educational programs
Requirements: • Bachelor’s degree in Social Work or a related field • Minimum two years’ experience in case management • In-depth knowledge of mental health policies and community resources • Valid Driver’s License and reliable transportation required • Bilingual candidates preferred
