This role focuses on delivering in-home support for families affected by neuro-cognitive disorders. Ideal candidates hold post-secondary education in health or social sciences and have at least two years of experience with older adults. You will develop connections within the district, making a lasting impact on caregivers and families navigating their journeys.
Key Responsibilities: • Provide in-home support and education services • Assist families in managing challenging behaviours • Collaborate with healthcare professionals and community partners • Conduct program evaluation and data collection • Travel within district areas as needed
Requirements: • Post-secondary education in relevant fields • Minimum 2 years experience with dementia care • Valid driver’s license and reliable vehicle • Bilingual in English and French required • Current CPR and First Aid Certificates
