Join the interprofessional team working with elderly patients experiencing dementia and complex behaviours. Your role includes assisting with daily living activities, ensuring patient safety, and reporting observations to nursing staff. The position supports the goal of assessing and stabilizing patients for successful transitions to long-term care or community.
Key Responsibilities:
• Assist patients with daily living activities and personal care
• Escort patients to appointments and maintain observation




• Report changes in patient conditions and behaviours
• Conduct security checks to ensure patient safety
• Document patient interactions and responses in clinical charts
Requirements:
• Personal Support Worker diploma required
• Current CPR – Level HCP certification
• Knowledge of the Mental Health Act and PHIPA
• Experience with Meditech preferred
• Availability for various shifts, including weekends
