This full time position at Providence Care Hospital tasks you with advancing geriatric care through your clinical expertise. You'll manage referrals and coordinate comprehensive assessments to optimize the delivery of care for our elderly patients. Key Responsibilities:
• Review and assess incoming complex referrals
• Conduct nursing assessments across multiple care domains
• Coordinate schedules to ensure efficient clinic operation




• Anticipate and fulfill patient care requirements
• Provide tailored education and counseling to patients
Requirements:
• Bachelor's Degree in Nursing
• Unrestricted registration with the College of Nurses of Ontario
• Three years’ clinical experience in geriatrics
• Two years in a clinical coordinator role
• Geriatrics training completion or commitment within 6 months
