The Minnesota Department of Human Services is seeking a SMRT Referral Specialist to assess referrals for eligibility and guide the SMRT process within Minnesota’s medical assistance programs. You will collaborate with counties and providers, prioritize cases, and ensure complete information and proper forms are requested, contributing to timely determinations and improved service delivery. This full time role offers day shifts and telework eligibility for qualifying Minnesota residents.
