Overview Kwanlin Dün First Nation (KDFN) Health & Wellness is seeking expressions of interest from qualified Nurse Practitioners to provide primary health care services at Natsékhi Kų̀ Health Centre. The Nurse Practitioner will work within a team-based Medical Home Model to improve access to timely, culturally safe, trauma-informed care for KDFN citizens and their families. At the Health Centre, the Nurse Practitioner will assess, diagnose, treat, and provide follow-up care for clients across the lifespan. This includes completing health histories and physical assessments, ordering and interpreting diagnostic tests, prescribing and managing medications, developing care plans, completing procedures within scope, and supporting care for acute, episodic, and chronic health concerns. The Nurse Practitioner will also support preventive care and health promotion, including screening, immunization support, chronic disease prevention and management, wellness planning, referrals, and client education. The role involves working closely with clients, families, community supports, and Health Centre staff to support coordinated, continuous care. The successful practitioner will bring strong clinical judgement, cultural humility, team-oriented practice, and a commitment to supporting First Nations health and wellness in a communitybased setting. Hiring preference will be applied in accordance with KDFN policy.
Qualifications Applicants must clearly demonstrate in their resume and cover letter how they meet the following qualifications:
Master of Nursing – Nurse Practitioner Stream, or equivalent Nurse Practitioner education
Recent experience providing primary care as a Nurse Practitioner in a community, clinic, or interdisciplinary health care setting
Strong clinical assessment, diagnostic, prescribing, treatment planning,
and follow-up skills across the lifespan
Demonstrated ability to provide culturally safe, trauma-informed, client- and familycentred care
Experience supporting preventative care, health promotion, chronic disease management, referrals, and continuity of care
Strong written, verbal, documentation, and interpersonal communication skills
Conditions of Employment
Current active registration, or eligibility for registration, with the Yukon Registered Nurses Association (YRNA) as a Nurse Practitioner in good standing
RCMP Security Clearance, including Vulnerable Sector Check
Tuberculosis screening
Valid Class 5 Driver’s Licence
Current Health Care Provider CPR, or equivalent
Working Environment
Providing care in clinic and home settings, including with individuals experiencing acute or communicable illness
Potential exposure to laboratory specimens, bodily fluids, or needle-stick injuries
Supporting clients who may be intoxicated, cognitively impaired, or experiencing emotional distress
Managing competing clinical and community-based priorities
Driving in adverse weather conditions, as required
Contractor Requirements
Providing proof of professional liability insurance appropriate to the scope of Nurse Practitioner services
Maintaining any required business, licensing, or professional registration requirements throughout the contract term
Meeting applicable tax, invoicing, and independent contractor responsibilities
Complying with KDFN policies, privacy requirements, documentation standards, and Health Centre procedures while providing services
