Key Responsibilities:



• Make appropriate support referrals for patients • Triaging referrals for Ontario Shores services • Facilitate admission processes to OSP • Communicate with referral sources effectively • Provide system navigation assistance to individuals

Requirements: • Registration as a Healthcare Provider or RN certificate • Current CPR-HCP certification • Valid driver's license • 3-5 years in psychiatric care • Knowledge of mental health legislation and community resources
