This role centers around medication reviews, optimization, and reconciliation as part of a agile team. Collaborate with GP Leads to develop recent prescribing strategies and provide informed recommendations on dosing and alternatives. You'll also assist with chronic disease management and play an essential role in training and educating staff, ensuring high standards of care are met.

Key Responsibilities:
Work with GPs on prescribing system policies
Optimize medication through structured reviews and consultations
Conduct medicines reconciliation as necessary




Educate colleagues about current prescribing guidelines
Support chronic disease management efforts within the practice

Requirements:
Relevant clinical pharmacy experience preferred
Solid understanding of medications and patient care
Excellent interpersonal and teaching abilities
Experience in team-based healthcare environments
Knowledge of cost-effective prescribing
