Engage in patient advocacy and healthcare navigation as a Clinical Guide Population Health RN with Rightway. This hybrid role in Denver focuses on triage, outreach, and care coordination to enhance member health outcomes.
Under the supervision of the Sr. Clinical Manager, this RN role emphasizes care planning, clinical education, and member support. You will work closely with high-risk members and guide them to optimal care solutions. Responsibilities include identifying barriers to care, conducting outreach, and leveraging data analytics for informed decision-making.
Key Responsibilities:
• Perform clinical triage using evidence-based protocols
• Conduct high-risk outreach to engage members proactively
• Assist members in scheduling appointments and referrals
• Document nursing assessments and care guidance accurately
• Serve as a trusted health advocate throughout care journeys
Requirements:
• Active RN license and degree (RN or BSN)
• 5+ years direct patient care experience
• Triage nursing experience preferred
• Population health or disease management experience required
• Robust expertise in care management and chronic support
