In this full-time role, you will become an integral part of the interprofessional team, providing best practices in diverse settings such as homes, schools, and physician practices. Your responsibilities include conducting assessments and developing personalized care plans to support clients in achieving their wellness goals.
Key Responsibilities:
• Assess and evaluate clients’ mental health and substance use
• Develop and implement individualized care plans
• Collaborate with interprofessional teams for optimal outcomes
• Provide ethical and safe care in various environments
• Support clients and families in their care journey
Requirements:
• Degree in Mental Health or related field
• Valid licensure as a Mental Health Practitioner
• Experience with addiction and mental health care
• Solid assessment and interpersonal skills
• Available for Monday to Friday daytime shifts
