Bayshore HealthCare seeks a passionate Social Worker to join its Integrated Care team in Toronto. This part time position focuses on delivering person-centered care to enhance clients' quality of life.

In this role, you will adapt social work principles for community settings, providing essential support to clients in North York and East York. Bringing compassion and care, you will work to maximize clients' emotional and social well-being while ensuring their voices are central to their care plans. Ideal candidates should possess strong interpersonal skills and community resource knowledge.

Key Responsibilities:
• Collaborate with clients and healthcare professionals
• Manage a caseload and prioritize service delivery




• Conduct assessments for social and emotional challenges
• Develop and evaluate psychosocial treatment plans
• Teach clients effective coping strategies

Requirements:
• Good standing with the Ontario College of Social Workers
• Preferred two years of therapy experience
• Proficient in English, French an asset
• Current driver’s license and access to a vehicle
• Valid CPR certification per policy requirements
