As the point of contact for a defined portfolio of primary care clinics, you will ensure successful implementation of innovative digital health solutions. Your day-to-day will involve engaging directly with physicians and clinic staff, coaching on effective practices, and monitoring success metrics to achieve targeted outcomes.
Key Responsibilities:
• Manage comprehensive digital transformations for 22 clinics
• Launch and support new clinic technologies effectively
• Be the primary resource for clinic practitioners
• Supervise timelines and readiness for tool implementations
• Coach on workflows and patient engagement strategies
Requirements:
• 2–4 years in client-facing healthcare roles
• Familiarity with Ontario FHO operational frameworks
• Strong ability to build trust with clinic staff
• Highly organized and self-motivated
• Knowledge of EMR and booking platforms as a plus
