Provide exceptional home nursing care as a Registered Practical Nurse with VHA Home HealthCare in the York region. Help clients live independently by being part of our Hospital Partnership Program.

VHA seeks part time RPNs who are passionate about delivering quality home health services in communities such as Woodbridge and Aurora. You will coordinate nursing care while evaluating and adjusting care plans effectively. With a diverse team backing you, your role will foster meaningful bonds with clients and families throughout their health journey.

Key Responsibilities:
• Assess client needs and plan individualized care
• Monitor ongoing patient conditions and outcomes
• Document care plans and quality of service
• Communicate effectively with clients and nursing supervisors
• Attend team meetings as needed

Requirements:
• Good standing with the College of Nurses of Ontario
• RPN diploma from an accredited institution
• Acute care experience preferred
• Valid First Aid/CPR Certification
• Commitment to lifelong learning and professional growth
