: Reporting to the Home Care Manager the Client Case Coordinator is responsible for the assessment of clients, coordination and evaluation of home care services and case management of region residents.

Coordinator will be responsible for long term care placement in rural under the Home Care program.

Coordinator will ensure that the delivery of home care and case management services is consistent with the policies, vision, mission and philosophy of the Saskatchewan Health Authority. "Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement"

Education
- Bachelor of Science in Nursing (BScN)
Other Education and Training
- Psychiatric Nursing diploma is acceptable
- Practical Nursing diploma with 4 years experience is acceptable
- Nursing diploma is acceptable
- Bachelor of Occupational Therapy is acceptable
- Bachelor of Physical Therapy is acceptable
- Bachelor of Psychiatric Nursing is acceptable
- Bachelor of Social Work is acceptable
