Detailed Overview

Under the general administrative direction of the Manager, Clinical Operations, the Clinical Coordinator, is a key member of the assigned interdisciplinary team. The Clinical Coordinator provides operational and clinical leadership, guidance and supervision to Disciplines Allied to Social Work and Social Work (DASW/SW) staff for the assigned program/service.

This incorporates the development and delivery of Disciplines Allied to Social Work and Social Work services throughout the assigned community programs, incorporating program planning, development, and quality improvement initiatives, as well as consulting and providing input to leadership regarding allocation of resources, and workload. Encourages knowledge exchange in day-to-day activities, and ensures clients and their natural supports are supported in navigating the health care system.

In collaboration with the Manager, Clinical Operations, assists in the recruitment and selection of staff; coordinates the onboarding and orientation of new hires; conducts and/or provides input to the Manager on staff performance appraisals; makes recommendations to the Manager on disciplinary action.

The Clinical Coordinator maintains linkages with other internal and external professionals, educational, and research resources and participates in research, risk management, and quality improvement and special projects initiatives; provides consultative and educational services to other professionals as well as acts as a clinical resource for the inter-disciplinary team on complex cases. Maintains knowledge of current developments in social work.

Responsibilities

- Coordinates the day-to-day workload to ensure optimal use of resources and efficient use of department staff by methods such as facilitating resolutions to complex situations, providing clinical guidance and advice, and collaborating with the Clinical Operations Manager as required.
- Establishes, implements, and evaluates the objectives, policies, procedures, standards, as they relate to Disciplines Allied to Social Work and Social Work, to meet assigned program goals and objectives in collaboration with the Manager, Clinical Operations.
- Participates in the recruitment and retention of recent staff by developing interviewing and orientation tools, pre-screens, and interviews candidates, supports selection process, onboarding, and orientating. Similarly supervises designated staff providing input to disciplining, directing, and carrying out performance evaluations of departmental personnel and organizing in-service education programs. Participates in the grievance process in consultation with Human Resources and the respective Manager. May manage other staff functions, such as timekeeping, work schedules, approval of vacation and leave of absence requests, as assigned.
- Determines workload measurements and other reports as required. Develops and maintains statistical data related to such items as, department activity levels, attendance, leave of absences and performance appraisals. Participates in program evaluation as requested.




- Represents the Disciplines Allied to Social Work and Social Work profession for the assigned program and establishes and maintains liaison with related departments, hospitals and community, agencies and associations. Maintains a close working relationship with other departmental personnel. Provides leadership in the development of the profession at the local and regional level.
- Coordinates the supervision of DASW and SW students by ensuring appropriate matching with field instructors, liaising with the university programs, and ensuring the proper documentation is in place, supporting the field instructors in observing the student’s client/family interviews, reviews of documentation and evaluation in accordance with program and university standards.
- Consults with the various programs regarding the impact of social issues on health (poverty, homelessness, discharge planning, etc.) providing information and resources to improve health care outcomes including the impact on length of stay.
- Enforces all safety regulation, instills safety awareness in department staff and follows up on all safety recommendations as required; ensures all department employees are updated on safety.
- Assumes a clinical caseload as required. Provides clinical services to patients as assigned by methods such as assessing and identifying risks and challenges, exploring the emotional psycho-social response to illness, and collaborating with interdisciplinary team to determine and implement optimal treatment planning.
- Provides education and in-service training to community agencies, Fraser Health staff and members of the interdisciplinary team by methods such as researching topics preparing and presenting material and documentation to facilitate learning goals, as requested. Leads assigned service educational programs as required.
- Maintains knowledge of current developments in evidence based social work practice by methods such as reviewing relevant literature, consulting with other professionals, attending relevant education programs and recommending new/changed practice standards to the Manager, Clinical Operations to enhance the delivery of patient care. Develops and carries out research activities in consultation with the interdisciplinary team and the assigned program with collaboration with the Manager, Clinical Operations and/or program leadership.
- Performs other related duties as assigned.

Qualifications

Education, Training & Experience Master's degree in Social Work from an accredited School of Social Work. Seven (7) years’ recent related professional clinical Social Work experience in community setting, including 3 years in a supervisory role in, or an equivalent combination of education, training and experience.





Current full registration with the BC College of Social Workers. Skills & Abilities

- Comprehensive knowledge of Social Work theory, and practice within a client and family-centred model.
- Comprehensive knowledge of the developmental stage in the individual and family life cycle and of psychosocial issues that may occur in response to changing health and dependency needs.
- Comprehensive knowledge of evidence-based practice related to clinical areas.
- Comprehensive therapeutic skills including clinical counselling, family counselling, substance use counselling, crisis and trauma counselling, brief therapeutic interventions, and loss and grief applicable to children, young adults and seniors, their families and support networks.
- Comprehensive knowledge of psychopharmacology, crisis theory, and de-escalation techniques.
- Comprehensive knowledge of DSM V Multiaxial Diagnosis or International Classification of Diseases.
- Knowledge and skill in working with culturally diverse and/or vulnerable populations, including sexual minorities, persons who use substances, and individuals with mental health concerns.
- Demonstrated knowledge and understanding of emotional and mental health issues related to trauma illness and injury, crisis intervention, protection of children and vulnerable adults and family cultural dynamics.
- Demonstrated proficiency in the use of a personal computer (PC) and applicable software including in the context of remote/virtual delivery of care.
- Comprehensive knowledge of organization policies, procedures and standards of care.
- Comprehensive knowledge of other health care disciplines and their role in client care.
- Comprehensive knowledge of research process and methodology.
- Demonstrated ability to provide leadership, work direction and training.
- Demonstrated ability to work collaboratively as a member of an interdisciplinary team.
- Demonstrated ability to communicate with (both verbally and in writing), and to deal effectively with co-workers, physicians, other health care professionals, clients and their families, and external agencies.
