Duties DUTIES: Reporting to the Social Services Supervisor, the Community Wellness Worker supports the health, wellbeing, and quality of life of MSIFN members, families, and the community through culturally grounded, prevention-focused, and community-based programs and services.

The position promotes holistic wellness by addressing the social, emotional, mental, physical, and cultural factors that contribute to individual, family, and community wellbeing. Working collaboratively within the Health & Social Services team, the role develops, coordinates, delivers, and evaluates wellness initiatives while providing outreach, education, advocacy, referrals, service navigation, and community engagement.

- Plan, coordinate, and deliver culturally grounded wellness programs that support healthy living, family wellbeing, and community safety.
- Work collaboratively with the Cultural Advisor to integrate traditional teachings, cultural practices, gatherings, and ceremonies into wellness programming where appropriate.
- Develop, implement, monitor, and evaluate prevention-based strategies that support healthy family dynamics and reduce risks related to family and community violence.
- Collaborate with the First Nation Child Welfare Representative, Post Majority and Prevention Worker,



and other internal and external partners to develop and deliver culturally grounded prevention strategies, coordinate services, facilitate referrals, and strengthen supports for families at risk of child welfare involvement.
- Assist individuals and families in identifying wellness-related concerns, strengths, needs, and goals and support the development of personalized wellness plans that are realistic, achievable, culturally respectful, and responsive to identified needs.
- Plan and implement community wellness events, workshops, sharing circles, outreach activities, awareness campaigns, educational initiatives, healing circles, parenting supports, community gatherings, wellness outings, healthy living initiatives, and other prevention-focused activities.
- Maintain knowledge of internal and external community resources, service providers, programs, and supports to assist with effective referrals and service navigation.
- Facilitate referrals and advocacy supports for individuals and families requiring access to shelters, mental health services, treatment programs, public health services, Ontario Health services, or other community-based supports.
- Work collaboratively with MSIFN staff and external partners to promote coordinated, respectful, and accessible services for members and community members.
- Maintain accurate, confidential, and up-to-date client records, program files, and service documentation in accordance with applicable privacy, case management, and administrative requirements.
- Develop program logic models, program plans, activity proposals, program-specific policies and procedures, reports, funding submissions, and updates for the Social Services Supervisor, Health & Social Services Manager, First Nation Manager, Chief & Council, and external funders as required.
- Prepare and monitor annual workplans and program budgets, track expenditures, prepare cheque requisitions where required, and provide financial and variance reporting in accordance with approved budgets and funder requirements.
- Participate in staff meetings, case management discussions, mandatory and program-specific training, after-hours/on-call rotation, adjusted work schedules, special projects, and other related duties as assigned.
- Comply with privacy legislation, laws, policies, and procedures while respecting and maintaining confidentiality; comply with health and safety legislation, policies, and procedures; work independently and collaboratively within a multidisciplinary team; and facilitate conflict resolution to maintain a positive and productive work environment.

PAY GRADE: $39.63–$45.41

Minimum Qualifications

- College diploma in Social Service Work, Community and Justice Services, Child and Youth Care, Indigenous Social Work, Mental Health and Addictions, Human Services, Community Development, or a related health and social services field.
- Bachelor's degree in Social Work (BSW),



Social Sciences, Indigenous Studies, Psychology, Community Development, Health Promotion, or a related field is preferred.
- Relevant training in one or more of the following areas: trauma-informed care, family violence prevention, mental health and addictions, crisis intervention, life skills coaching, or case management.
- Life Skills Coaching Certificate is considered an asset.
- Minimum three (3) years of progressively responsible experience supporting individuals, families, youth, Elders, and communities through health, social services, community wellness, prevention programming, outreach, case management, advocacy, or related human services.
- Minimum five (5) years total work experience.
- Knowledge and understanding of Indigenous culture, traditions, teachings, community dynamics, and culturally grounded approaches to wellness and service delivery.
- Knowledge of child welfare systems, youth services, post-majority supports, family wellness, and legislation affecting First Nations communities.
- Knowledge of community wellness, family violence prevention, healthy living, primary prevention, and strength-based approaches to supporting individuals and families.
- Strong interpersonal and communication skills, with the ability to build trust, maintain respectful relationships, and work effectively with members, families, colleagues, and community partners.
- Ability to support individuals and families with care, confidentiality, cultural sensitivity, sound judgment, and appropriate professional boundaries.
- Strong planning, organizational, and time management skills, with the ability to manage multiple priorities, programs, cases, reports, and deadlines.
- Ability to develop, deliver, monitor, and evaluate community-based programs, workshops, outreach activities, wellness plans, and prevention initiatives.
- Knowledge of budgeting, work planning, expenditure tracking, variance reporting, funding proposals, and funder reporting requirements.
- Proficiency using Microsoft Office, electronic documentation systems, case management tools, email, internet, and other technology required to support accurate records, reporting, communication, and service delivery.
- Valid driver's license and $1M liability insurance.
- CPIC/VSS acceptable to position upon conditional offer.
- Work experience with a First Nation or Indigenous Organization is preferred.
- An equivalent combination of education, training, and experience, as deemed appropriate by the hiring panel, will also be considered.
