Purpose of the Position:
This position is part of a non-police intervention project where a multi-disciplinary team skilled in de-escalation and crisis intervention supplies de-escalation and outreach in Toronto’s Downtown East. The team provides consistent, rapid response, short-term follow-up support, and proactive outreach to people experiencing marginalization. The team’s focus is on immediate intervention with clients (de-escalating situations, mitigating crises, and performing activities like wellness checks, crisis counselling, safety assessments, or nonviolent conflict resolution), while the overall goal is to help build safe and inclusive communities.
The Registered Nurse collaborates with members of the team to provide de-escalation, engagement, outreach, and referral to marginalized individuals. Additionally, the Nurse will offer assessment and triaging of complex health needs, elevated risk for acute health emergencies, mental health and substance use challenges.
The Registered
Nurse will use a holistic, client-centred approach to providing care to marginalized individuals with an emphasis on assessment, acute treatment (i.e. wound care), disease prevention, harm reduction and health promotion.
This project will incorporate a trauma-informed understanding of marginalized communities, including a strong analysis of anti-Indigenous and anti-Black racism in all aspects of the project, from hiring and project development to implementation and data collection.
Roles and Responsibilities:
§ Provides nursing assessment and treatment and/or counselling interventions to a marginalized population presenting with complex health needs.
These may include (often previously untreated) wounds, skin disorders, infectious diseases, and mental health issues.
§ Provides individual client assessments (physical, emotional, and psychological), including obtaining client history, monitoring vitals, and triaging of clients based on level of need.
§ Collaborates with clients, health care providers, and community support workers to develop appropriate, client-centred, individualized care plans. [NB1]
§ Engages clients in risk factor and health problem identification, goal setting and making choices of intervention for disease treatment, prevention, and health promotion.
§ Undertakes outreach services in a fast paced and changing environment.
§ Keeps complete and accurate records of client contacts and care through timely documentation.
§ Contributes to the development of and implements strategies that promote health, prevent disease, and foster independence and healthy living.
§ Contributes to the development of learning resources and participates in health education.
§ Actively participates in team meetings as appropriate/required.
§ Provide feet-on-the-street outreach, delivering clinical and crisis services to clients in the community and outdoors.
QUALIFICATIONS, SKILLS and EXPERIENCE:
§ Bachelor of Science in Nursing or Diploma in Practical Nursing
§ Current Registration and member in good standing with the Ontario College of Nurses as a Registered Nurse OR a Registered Practical Nurse.
§ Current Membership with the RNAO and evidence of liability insurance.
§ 3 years of clinical experience in working with clients in community-based health/primary care settings.
§ Experience working with homeless or marginalized service users preferred and with client populations with serious and persistent mental illness and/or addiction.
§ Knowledge of and commitment to client-centred philosophy and basic principles and practice of community service delivery and chronic disease management models.
§ Knowledge of community resources relevant to health promotion and illness prevention.
§ Open-minded, non-judgmental with a commitment to upholding the rights and dignity of all people and respecting their diversity.
§ Knowledge and proficiency in current, evidenced-based methods and practices in health care delivery, with an emphasis on mental health, health promotion and risk reduction.
§ Proficiency in computer software, particularly in Microsoft Word, Excel, Outlook.
§ A sound understanding of and commitment to the Social Determinants of Health, and the ability to effectively participate in the efforts of an interdisciplinary team to contribute to comprehensive health care.
