Rehab is seeking a dedicated nutrition professional to join our interdisciplinary team and make a meaningful impact on the lives of our residents every day. If you’re passionate about clinical nutrition, resident-centered care, and collaborating with an outstanding healthcare team, we’d love to meet you! As our Registered Dietitian, you’ll play an essential role in improving resident health outcomes by providing expert nutritional assessments, care planning, education, and oversight of dietary services.
Complete nutritional initial, quarterly, annual, and significant change assessments in accordance with federal and state regulations. Conduct monthly nutrition reviews for high‑risk residents, including: Hemodialysis residents Develop, implement, and monitor individualized nutrition care plans. Coordinate nutrition services with the Dietary Supervisor and interdisciplinary care team.
Monitor dietary operations for compliance with nutrition, food safety, sanitation, and quality standards. Inspect diet trays to ensure physician diet orders are accurately followed. Consult with physicians, nurse practitioners, dialysis dietitians, and other healthcare providers regarding resident nutritional needs.
Participate in daily stand‑up meetings and interdisciplinary care conferences.
Provide leadership and support to the Dietary Department while promoting excellence in resident care.
Registered
Dietitian with the Commission on Dietetic Registration (CDR) or eligible to sit for the RD Exam Licensed Dietitian in the State of California (or eligible as required) Bachelor's Degree in Food and Nutrition or related field Remain in good standing with the California Department of Public Health One year of clinical healthcare or skilled nursing experience Completion of an accredited dietetic internship Solid clinical nutrition knowledge Excellent assessment and care planning skills Knowledge of federal and California skilled nursing regulations Frequent standing and walking throughout resident units and kitchen areas Frequent documentation and computer use Ability to communicate effectively with residents, families, physicians, and staff This position works in a skilled nursing and rehabilitation setting with a low to moderate noise level.
