Location: Various HRFHT Sites - North York/Toronto Area

Compensation: $77,673 annually

Benefits: Comprehensive benefits package (health, dental, vision, EFAP) and HOOPP defined benefit pension plan

Schedule: 5 days a week, including evening and occasional Saturday coverage as required

About the Organization The Humber River Family Health Team (HRFHT) is a non-profit organization providing interdisciplinary, patient-centred primary care across northwest Toronto. Our team works collaboratively to improve access to high-quality, inclusive primary care, chronic disease management, health promotion, and preventative services.

Join our growing team! We are hiring 2 Full-Time Permanent Registered Nurses (RNs).

About the Position

HRFHT is seeking 2 Registered Nurses to support primary care access, recent-patient intake, screening, triage, care coordination, and patient navigation. The RN will work across designated HRFHT sites and programs, helping patients access the right level of care and connecting them with internal and community services.

The role also contributes to patient attachment and rostering processes, program development, program facilitation, health promotion, and quality improvement. The RN is expected to exercise independent nursing judgment within scope and to lead or support triage, care coordination, and clinical follow-up as appropriate.

Accountability The position reports directly to the Supervisor, Clinical Services.

Key Responsibilities

- Complete new-patient intake, comprehensive and focused nursing assessments, and health screening.
- Triage patient concerns, determine urgency and appropriate next steps, and escalate to a Nurse Practitioner, physician, or emergency services when clinically indicated.




- Support new-patient attachment and rostering by identifying health needs, outstanding preventative care, chronic disease needs, and appropriate follow-up.
- Navigate patients to HRFHT programs, primary care providers, specialists, community agencies, hospital services, and other health and social supports.
- Coordinate care and follow-up for patients with complex health or social needs, including communication with internal and external providers.
- Provide nursing care within RN scope, including chronic disease support, preventative care, immunizations, health teaching, medication-related support, and clinical monitoring.
- Develop, implement, facilitate, and evaluate clinical, health-promotion, disease-prevention, and community-based programs.
- Facilitate individual and group education sessions and support patients with self-management and system navigation.
- Identify gaps in care, access barriers, and emerging patient or community needs and contribute to service and workflow improvements.
- Participate in quality improvement, clinical pathway development, care protocols, program evaluation, and data collection.
- Maintain accurate and timely documentation in the EMR and support appropriate referral and follow-up processes.
- Work collaboratively with physicians, Nurse Practitioners, RPNs, allied health professionals, reception/intake staff, and community partners.
- Participate in interdisciplinary meetings, case reviews,



organizational initiatives, and other duties as assigned.

Qualifications

- Current registration in good standing with the College of Nurses of Ontario as a Registered Nurse.
- Demonstrated experience in primary care, community health, ambulatory care, or a related clinical setting.
- Strong assessment, triage, clinical decision-making, care coordination, and patient-navigation skills.
- Knowledge of preventive care, chronic disease management, health promotion, and community resources.
- Experience developing or facilitating clinical, health-promotion, education, or community-based programs is an asset.
- Ability to work independently while recognizing when consultation or escalation is required.
- Strong communication, facilitation, organizational, and interdisciplinary collaboration skills.
- Experience with electronic medical records is an asset.
- Demonstrated flexibility and ability to work in a growing, evolving primary care environment.
