Location: Various HRFHT Sites - North York/Toronto Area
Compensation: $55,686 annually
Benefits: Comprehensive benefits package (health, dental, vision, EFAP) and HOOPP defined benefit pension plan
Schedule: 5 days a week, including evening and occasional Saturday coverage as required

About the Organization The Humber River Family Health Team (HRFHT) is a non-profit organization providing interdisciplinary, patient-centred primary care across northwest Toronto. Our team works collaboratively to improve access to high-quality, inclusive primary care, chronic disease management, health promotion, and preventative services.
Join our growing team! We are hiring 3 Full-Time Permanent Registered Practical Nurses (RPNs).
About the Position
HRFHT is seeking 3 Registered Practical Nurses to support primary care access, new-patient intake, screening, triage, care coordination, and patient navigation. The RPN will work across designated HRFHT sites and programs, helping patients access the right level of care and connecting them with internal and community services.
The role also contributes to patient attachment and rostering processes, program development, program facilitation, health promotion, and quality improvement. The RPN works within their scope of practice and established clinical processes, consulting and escalating to an RN, Nurse Practitioner, or physician when patient complexity, predictability, or clinical risk requires additional assessment.
Accountability The position reports directly to the Supervisor, Clinical Services.
Key Responsibilities
- Complete new-patient intake, focused nursing assessments, health screening, and collection of relevant clinical information within RPN scope.
- Support patient triage by identifying presenting concerns, recognizing changes in condition, applying established protocols,
and escalating to an RN, Nurse Practitioner, physician, or emergency services when required.
- Support new-patient attachment and rostering by identifying health needs, preventative care gaps, chronic disease needs, and required follow-up.
- Navigate patients to HRFHT programs, primary care providers, community agencies, hospital services, and other health and social supports.
- Support care coordination and follow-up for patients, including communication with the interdisciplinary team and community partners.
- Provide nursing care within RPN scope, including health teaching, immunizations, chronic disease support, clinical monitoring, and preventative care activities.
- Participate in the development, implementation, facilitation, and evaluation of clinical, health-promotion, disease-prevention, and community-based programs.
- Facilitate patient education and group programming and support patients with self-management and health-system navigation.
- Identify access barriers, service gaps, and patient needs and bring concerns forward to the clinical team to support service improvement.
- Participate in quality improvement activities, data collection, referral processes, and program evaluation.
- Maintain accurate and timely documentation in the EMR and ensure follow-up tasks are communicated to the appropriate team member.
- Work collaboratively with physicians, Nurse Practitioners, RNs, allied health professionals, reception/intake staff, and community partners.
- Participate in interdisciplinary meetings, case reviews, organizational initiatives,
and other duties as assigned.
Qualifications
- Current registration in good standing with the College of Nurses of Ontario as a Registered Practical Nurse.
- Experience in primary care, community health, ambulatory care, or a related clinical setting is preferred.
- Strong focused-assessment, patient-engagement, care-coordination, and service-navigation skills.
- Ability to recognize changes in patient condition, apply established triage processes, and appropriately consult or escalate care.
- Knowledge of preventative care, chronic disease support, health promotion, and community resources.
- Experience developing or facilitating health-promotion, education, or community-based programs is an asset.
- Ability to work independently within scope and collaboratively as part of an interdisciplinary team.
- Strong communication, facilitation, organizational, and documentation skills.
- Experience with electronic medical records is an asset.
- Demonstrated flexibility and ability to work in a growing, evolving primary care workplace.
