The Colorado Coalition for the Homeless is hiring a Case Manager I to work with families, children, and individuals experiencing homelessness or at risk. You will coordinate services, develop care plans, monitor progress, advocate, and maintain case documentation within an integrated health team.

Location is Denver, CO with a focus on delivering outreach, referrals to mainstream services, and crisis intervention while applying housing-first and trauma-informed care practices.
