Position Summary

Reporting to the Home and Community Care (HCC) Nurse Manager and Assistant manager, the HCC/Public Health Lead collaboratively develops and implements the HCC/PH program, addressing the physical, social, emotional, and spiritual care needs of clients and their caregivers. HCC/Public Health Lead will work with the HCC Program by delivering public health programs and keeping HCC staff informed of updates and changes. HCC/PHL coordinates and implements in-home nursing services, works with a team of Personal Support Workers (PSW’s) and other health care professionals.

This individual will provide client-centered nursing services to HCC clients in various KO communities. The HCC/Public Health Lead will fulfill responsibilities in accordance with the standards of practice and guidance of the College of Nurses of Ontario. The HCC/Public Health Lead will promote the mission, vision, and values of the KO organization.

Duties and Responsibilities

- Assesses, plans, implements and evaluates nursing programs and community-wide based initiatives using the current knowledge of practices and trends related to the field of public health.
- Acts as a resource person to communities and HCC team.
- Provides health education, counseling, supports and advocates for healthy public policy and legislation while working within the policies of the program and those in the KO communities.
- Establishes effective working relations with peers, appropriate professional agencies and community groups.




- Participates in interagency committees/community coalitions as appropriate.
- Identifies local health issues, needs and participates in population-needs-based health planning and evaluation.
- Contributes to the development and the implementation of the Home and Community Care Service Delivery Plan.
- Maintains competence relevant to current practice and trends in the field of public health by participating in continuing education opportunities.
- Oversee the Personal Support Workers on the HCC team to support them and provide education/teaching. Support HCC team with cEMR Telus charting system.
- Performs other duties as assigned by Nurse Manager, Director of Health and Director of Nursing.

Education and Experience

- Registered Nurse (RN) certification in valuable standing with the College of Nurses of Ontario.
- Current CPR/BLS certification
- Advanced Foot Care certification or must be willing to obtain.
- A minimum of 2 years full time recent clinical practice work experience in emergency or primary health care. Recent northern nursing experience is an asset.
- Clinical practice experience in remote or isolated communities and with First Nations populations are considered an asset.
- Good interpersonal and communication skills.
- Demonstrated knowledge of Public Health for communities.
- Ability to speak Cree, Oji-Cree, or Ojibway is an asset.
- Excellent report writing skills and computer skills
- Willingness and ability to travel 1-2 weeks at a time.
- Valid Ontario’s driver license.
