Job Summary

Duties
- Deliver direct patient care in various settings such as hospitals, nursing homes, and home health environments, ensuring safety and comfort.
- Administer medications accurately following prescribed protocols and document medication administration in compliance with healthcare standards.
- Assist with daily activities including bathing, dressing, mobility support, and nutritional needs to enhance patient quality of life.
- Conduct clinical procedures such as phlebotomy (blood draws), vital signs monitoring, and wound care with precision and professionalism.
- Utilize electronic health record systems like Epic or Cerner to document patient information, treatment plans, and progress notes efficiently.
- Collaborate with physicians, nurses, case managers, and other healthcare professionals to develop and implement personalized care plans.
- Provide specialized care for patients with dementia, hospice needs,
pediatrics or senior care requirements while maintaining dignity and respect.
- Support laboratory testing procedures and assist in pain management strategies to optimize patient comfort.
- Engage in case management activities to coordinate services for complex or long-term patients ensuring seamless continuity of care.
Skills
- Proven experience working in nursing homes, hospitals or acute care settings with a strong understanding of medical terminology.
- Skilled in medication administration, patient assessment, and documentation within electronic health record systems such as Epic or Cerner.
- Knowledgeable in dementia care, hospice services, pediatrics, senior care and home health environments.
- Experience working with children and elderly populations with a compassionate approach to diverse patient needs.
- Proficient in phlebotomy techniques and laboratory procedures; familiarity with pain management practices is a plus.
- Strong case management abilities with the capacity to coordinate multidisciplinary healthcare services effectively.
- Excellent communication skills for collaborating with medical teams and educating patients about their treatments.
- Ability to adapt quickly to different healthcare settings while maintaining high standards of patient safety and quality of care.
