In this vital position, you will lead the Aging at Home program, offering essential services that enable seniors to live independently. Your responsibilities will include assessing client needs, coordinating home support services such as cleaning and transportation, and maintaining crucial program records. You will also engage with outside agencies for effective service delivery tailored to each client’s requirements.
Key Responsibilities:
• Oversee Aging at Home service delivery philosophy
• Conduct assessments for client service needs




• Maintain tracking of lawn care and snow removal services
• Facilitate access to gas cards for medical appointments
• Document client services in OHATS and Excel
Requirements:
• 1+ years in client service experience
• Experience with older populations (55+)
• Proficiency in Microsoft Office Suite
• Background in non-profit volunteer work
• Relevant degree/diploma considered an asset
