You will lead psychosocial assessments, contribute to comprehensive treatment plans, and engage with a multidisciplinary team to enhance patient experiences and outcomes. Key Responsibilities:
- Deliver psychosocial assessments for patients with neurological disorders
- Formulate comprehensive treatment strategies within a team
- Actively participate in multi-disciplinary care coordination
- Follow up with patients via telehealth and in-person visits

- Master’s Degree in Social Work is needed
- Registration with Ontario College of Social Workers required
- At least four years in clinical practice
- Skills in cognitive behavioral therapy are advantageous
