Full Time Intensive Case Manager Position (ICM), 1 year contract.

Summary:
The Intensive Case Manager role is designed to support individuals with a person centred and trauma informed approach who have more complex needs in their search for low-barrier housing.
Housing Support Worker Duties & Responsibilities:
● Participate in monthly case conferencing
● Creation and maintenance of accurate client case files and notes
● Provide ICM support to individuals in Dartmouth who are facing barriers to housing (including but not limited to for example; addictions, a history with corrections, mental health conditions, physical disabilities, etc.)
● Assist clients to search and apply for suitable, affordable housing, and provide support to the individual after housing has been secured (follow-up/continuing care)
● Assist clients with securing funding for moves and provide advocacy for setting up power, and/or referrals other agencies as appropriate
● Assist in maintaining housing stability of Welcome Housing clients in need of higher levels of support in order to ensure maintenance of housing stability by working with tenants and landlords to maintain clients in their present units (eviction prevention)
● Provide at risk individuals with a variety of basic life skill supports and links to other support services that reduce the risk of homelessness and eviction
Qualifications The ideal candidate will possess the following skills and / or credentials:
● Education and/or experience in health education/promotion, human services, social work or related field;
● Working knowledge of the determinants of health and services available within the community that impact upon an individual’s ability and/or opportunity to move beyond personal and systemic barriers;
● Knowledge of issues and resources in the community related to homelessness, poverty, harm reduction, mental health and addiction issues, community development and capacity building;
● Excellent problem solving skills and understanding of the barriers that deter at-risk individuals from obtaining and maintaining safe and affordable housing;
● Strong team player and ability to work independently;
● Good organizational, interpersonal, written and verbal communication skills;
● Computer skills and report writing ability required
This is a unionized position, employees are required to reference the collective agreement implemented by SEIU2 and the employer.
Must have access to a reliable vehicle and be able to use this vehicle during working hours.

Pay: $60,770.00 per year
