Job Summary
Join our dedicated healthcare team as a Licensed Practical Nurse (LPN), where your compassionate patient care and clinical expertise will make a meaningful difference in the lives of diverse patient populations. In this vital role, you will provide high-quality nursing services across various settings, including long-term care facilities, hospitals, home care, and assisted living communities. Your energetic approach and attention to detail will support patient well-being, medication management, and health education while collaborating with multidisciplinary teams to ensure optimal outcomes.
Duties
- Deliver comprehensive patient care by monitoring vital signs, administering medications, and assisting with daily activities.
- Support patients with dementia, senior care, hospice needs, pediatrics, and acute conditions through personalized nursing interventions.
- Perform clinical procedures such as phlebotomy, laboratory specimen collection, and wound care with precision and professionalism.
- Utilize electronic health record systems like Epic and Cerner to document patient information accurately and efficiently.
- Educate patients and families on medication regimens, disease management, and health promotion strategies.
- Assist in case management activities by coordinating care plans and communicating with healthcare providers.
- Maintain a safe workplace by adhering to infection control protocols and medical terminology standards.
Qualifications
- Valid state licensure as a Licensed Practical Nurse (LPN) with current certification.
- Prior experience in nursing home settings, hospitals, or acute care environments is highly preferred.
- Demonstrated expertise in medication administration, pain management, and medical terminology.
- Experience working with diverse populations including seniors, children, and hospice patients.
- Skills in phlebotomy, laboratory procedures, and medical office operations are advantageous.
- Familiarity with electronic health records such as Epic or Cerner is desirable.
- Strong communication skills coupled with a compassionate approach to patient care are essential for success in this role.
