Your Opportunity:
As a Licensed Practical Nurse (LPN) and Lead Service Provider (LSP), you will be part of an interdisciplinary mental health team supporting individuals who are subject to Community Treatment Orders (CTOs). Working under the direction of the CTO Coordinator and Clinical Supervisor, you will provide recovery-oriented, client-centered care to support stability, wellness, and community integration. In this role, you will conduct client outreach, ongoing monitoring, and nursing assessments within the LPN scope of practice. You will complete biopsychosocial, mental health, and physical health assessments, monitor client progress, and assess risks related to self-harm, suicidal ideation, homicidal ideation, and other safety concerns. You will identify changes in client condition and collaborate with the care team to ensure appropriate interventions and follow-up. Responsibilities also include supporting medication management and administering prescribed medications, including depot injections and other treatment modalities as ordered by the physician and in accordance with professional standards and organizational policies. You will provide a scope appropriate case management and care coordination, assist clients in accessing community resources and supports, and work collaboratively with psychiatrists, physicians, community partners, and other healthcare professionals. The successful candidate will demonstrate strong adaptability skills, clinical judgment, communication, and organizational skills, with the ability to work independently while utilizing available consultation and support resources.
You will maintain accurate clinical and legal documentation and contribute to protected, high-quality, patient-centered care consistent with the values of Recovery Alberta.

Required Qualifications: Completion of an accredited practical nurse education program. Active or eligible for registration and practice permit with the College of LPNs and HCAs of Alberta (CLHA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).
Additional Required Qualifications:
Demonstrated competence in medication administration, including injectable medications and depot injections, in accordance with physician orders, professional standards, and best practice guidelines. Strong assessment skills, including mental health, psychosocial, risk, and physical health assessments, with the ability to identify and respond appropriately to changes in client status. Demonstrated ability to work independently in a community-based setting, effectively managing a caseload, prioritizing competing demands, and exercising sound clinical judgment with minimal supervision. Strong commitment to ethical practice, professional accountability, integrity, confidentiality, and respectful therapeutic relationships. Excellent communication, documentation, organizational, and interpersonal skills. Proven ability to collaborate effectively within an interdisciplinary team while maintaining autonomy in day-to-day practice.
Experience maintaining accurate clinical and legal documentation and providing recovery-oriented, patient-centered care. Ability to recognize situations requiring consultation or escalation and take appropriate action to support safe and effective client care.
Preferred Qualifications:
3-5 years of adult mental health experience within the last 5 years. Experience working with individuals with severe and persistent mental illness in community-based settings. Knowledge of the community resources. Understanding of mental health legislation, particularly Community Treatment Orders. Experience in mental health case management, outreach, risk assessment, care coordination, and proficiency with Connect Care (Epic). Having completed NVCI, Non-violent Crisis Intervention course.
