Experience Currently registered in good standing with the British Columbia College of Nurses and Midwives (BCCNM) Graduation from an accredited Practical Nurse Program Minimum of two (2) years nursing experience in long term care is preferred Key Responsibilities Provide direct nursing care, including administration of medications, medical treatments and documented procedures as per provincial regulations, professional standards of practice and facility policies and procedures. Evaluate the effectiveness of nursing care plans and treatments Assist with admissions, discharges and transfers of residents Ensure the appropriate clinical assessment of residents is completed and followed up on through a care plan or planned intervention. Assessments include, but are not limited to Braden Scale, SLUMS, Morse Fall Risk, basic vital signs, neuro vital signs, medication reviews and Care Conferencing processes.
Develop and implement individualized care plans and ensure the plans are implemented. Liaises with Residents’ families,
updating them with any changes in health status, medication changes, or personal care needs.
