Shifts: 5 on - 2 off

Schedule: 0700 - 1500 (can be flexible with start and finish times)

Wage:

- $43.41 per hour starting

Full-Time Leadership Opportunity: Are you an experienced Licensed Practical Nurse who is passionate about leadership, clinical excellence, and improving the quality of care for seniors?

The Vineyards Residence is seeking a Clinical Lead (LPN) to join our leadership team. This is an exciting opportunity for an experienced nurse ready to move beyond bedside care into a leadership role where you will help shape clinical practice, mentor nursing teams, oversee quality initiatives, and support the daily operations of our nursing department.

Working closely with the Director of Care and Executive Director, the Clinical Lead plays an integral role in ensuring exceptional resident care while fostering a culture of accountability, collaboration, and continuous improvement.

Our residence:

Opened in 2021, The Vineyards Residence is a state-of-the-art five-storey, 134-suite residence offering specialized Enhanced Assisted Living and Memory Care services to the Okanagan region.

Our residence is designed around small neighbourhoods of 16–17 suites, creating a comfortable home-like environment where residents develop meaningful relationships with their caregivers and neighbours.

Position Summary:

The Clinical Lead (LPN) provides clinical leadership and operational support to the nursing department while working closely with the Director of Care and Executive Director to ensure the delivery of exceptional resident-centered care.

This supernumerary leadership role oversees clinical operations, quality improvement initiatives, resident care coordination, documentation standards, audits, and regulatory compliance while supporting nursing staff in the delivery of secure, consistent, and compassionate care.

The Clinical Lead serves as a key clinical resource for Licensed Practical Nurses, Health Care Aides, physicians, allied health professionals, residents, and families, promoting accountability, collaboration, and excellence in clinical practice throughout the residence.

Key Responsibilities

Clinical Leadership:





- Support the Director of Care with daily clinical operations.
- Provide leadership, guidance, and support to nursing staff across all neighbourhoods.
- Conduct daily clinical rounds and respond to resident care concerns.
- Provide clinical support during complex resident situations.
- Assist with staffing coordination and provide floor coverage when required.
- Lead weekly nursing huddles, HCA huddles, wound rounds, and interdisciplinary meetings.

Resident Care Coordination:

- Coordinate and facilitate resident care conferences.
- Schedule annual, requested, and admission care conferences.
- Collaborate with physicians, families, and interdisciplinary team members.
- Support resident assessments and care planning.
- Monitor residents with wounds, aggressive behaviours, catheters, ostomies, restraints, and other clinical risk factors.
- Ensure follow-up actions from care conferences are completed.

Clinical Quality & Documentation:

- Review daily 24-hour reports and clinical documentation.
- Maintain communication boards and clinical tracking tools.
- Ensure completion of required documentation, including:
- MAR/TAR documentation
- Nursing Measures
- PAINAD assessments
- Blood pressure monitoring
- PPS scores
- Care plan updates
- Review flagged charts and physician communications.
- Support PointClickCare documentation standards.

Quality Improvement:

- Complete and oversee:
- MAR/TAR audits
- Medication room audits
- Resident care audits
- Care plan audits
- Narcotic audits
- Restraint audits
- Monthly vital sign and weight audits
- Identification band audits
- Admission audits
- Hospital transfer audits
- Monitor quality indicators and implement improvement initiatives.
- Support accreditation and regulatory readiness.

Staff Support:

- Mentor and support LPNs and HCAs.
- Reinforce organizational policies and best practices.
- Assist with orientation of new nursing staff.
- Identify opportunities for ongoing education and competency development.
- Promote teamwork and a positive workplace culture.

Communication & Collaboration:

- Work collaboratively with leadership, physicians, pharmacy, rehabilitation services, and interdisciplinary team members.




- Act as a liaison between frontline staff and leadership.
- Communicate resident concerns and quality improvement opportunities.
- Foster respectful and skilled relationships with residents and families.

Health & Safety:

- Promote infection prevention and control practices.
- Ensure compliance with Occupational Health & Safety policies.
- Participate in emergency preparedness and response.
- Support safe clinical practices throughout the residence.

Qualifications:

- Current Licensed Practical Nurse (LPN) registration with BCCNM in good standing.
- Minimum five years of experience in long-term care, assisted living, or gerontological nursing.
- Previous leadership, charge nurse, Resident Care Coordinator, or supervisory experience preferred.
- Experience with PointClickCare or comparable electronic documentation systems.
- Current CPR and First Aid Certification.
- Criminal Record Check – Vulnerable Sector.
- Experience supporting residents living with dementia and complex care needs.

- Commitment to resident-centered care and continuous quality improvement.
- Working knowledge of long-term care regulations and clinical standards.

Minimum five years of experience in long-term care, assisted living, or gerontological nursing
