The Nurse Practitioner (NP) in the Pediatric Cancer Survivorship program will function in an advanced nursing role, demonstrating a high level of autonomy and expert skill to formulate clinical decisions and provide management and surveillance for late effects of childhood cancer. They will provide outpatient care for patients with a history of childhood cancer, including brain tumors, solid tumours, and leukemia/lymphoma. Duties
Work with Pediatric Cancer Survivors in 2 Survivorship Clinics Utilize the Children's Oncology Group guidelines to create surveillance recommendations and follow-up plans Disseminate recommendations and plans to primary care providers Be responsible for organising surveillance investigations, following up on results and initiating referrals to specialists Have a basic understanding of fertility and the potential impact from cancer treatment Work with a multidisciplinary team to provide comprehensive care Collaborate with the SickKids Aftercare Team and attend 2 Transition Clinics each year Assist with reviewing referrals from other providers to access the Survivorship Clinic and be responsible for referring patients to other Survivorship Clinics across Canada when needed Work primarily in-person with the occasional virtual clinic visit Other responsibilities will include academic and leadership activities, including delivering, coordinating, and participating in research, education and professional development activities. Requirements
Completion of a Master's degree in Nursing required Current registration with the College of Nurses in Ontario in the Extended Class Experience with the Adolescent and Young Adults (AYA) population Strong organisational, leadership and collaborative practice skills Proven ability to work autonomously and manage complex clinical situations Experience in quality improvement and program development preferred Effective verbal and written communication skills, with demonstrated sensitivity to patient and family needs Proficiency in information technology and virtual care platforms Membership in relevant qualified organisations (eg.
