Key Responsibilities:
• Collaborate on Community Member care planning and oversight
• Conduct scheduled wellness assessments and evaluations




• Develop and review individualized Care Plans
• Lead care conferences with families and healthcare providers
• Coordinate health services and maintain documentation
Requirements:
• Registered Practical Nurse (RPN) in good standing
• Minimum of 3 years’ nursing experience in senior care
• Strong assessment and planning skills
• Knowledge of Microsoft Office applications
• CPR/First Aid Certification required
