Lakeview Manor
This position is fully in office, 5 days a week located at Lakeview Manor in Beaverton, Ontario.
IPAC Practitioner
- Develop, lead and evaluate the infection prevention and control (IPAC) program, adhering to best practices and policies within the region
- Perform daily surveillance activities by collecting, analyzing and communicating epidemiologic data
- Conduct epidemiologic investigations, manage and coordinate activities during an outbreak or isolation events, and integrate findings into formal plans for IPAC
- Formulate action plans to manage and respond to infection outbreaks within the homes
- Chair the IPAC Committee
- Create and maintain Resident Infection Profiles to help identify and communicate risks and treatments for all residents within the home
- Liaise and collaborate with Public Health to identify reportable diseases
- Communicate and notify the Home's Leadership Team to advise of risks, declared outbreaks and necessary precautions required in order to respond to incidences of infection
- Be responsible for monthly, quarterly and annual reporting of infection control data and activities
- Conduct risk assessments and audits to evaluate the effectiveness of current programs and plans, and assist with the development of short and long term goals
- Create, maintain and audit resident immunization records,
as well as screen and track resident Antibiotic Resistant Organism (ARO) upon admission and re-admission to the homes
- Conduct immunization clinics for residents, as required
- Administer and document staff immunizations and screening tests in accordance with policies and procedures
- Manage and coordinate the Antibiotic Stewardship Program (ASP)
- Develop and implement IPAC policies and procedures based on currently accepted based practice and legislative and/or program requirements
- Develop and oversee the hand hygiene program
The successful applicant will have:
- A 4-year Undergraduate Degree specific to Public Health, Epidemiology, Microbiology, Nursing or other related discipline
- Completion of an Infection Prevention and Control Canada endorsed IPAC course
- Current certification in infection control from the Certification Board of Infection Control and Epidemiology (CIC); those actively working towards certification may alsobe considered and will be required to obtain certification by no later than March 2025
- Membership in IPAC Canada
- Several years of experience in infection control, with previous experience working in a long-term care facility preferred
- Sound knowledge of epidemiological principles, statistics, microbiology, infectious diseases, asepsis, disinfection/sterilization, immunization, hand hygiene, pandemic and emergency planning
- Knowledge of Public Health Regulations, Registered Health Professionals Act, Provincial Infectious Diseases Advisory Committee (PIDAC), Long Term Care Act and Regulations, Occupational Health and Safety Act, and privacy laws
- Demonstrated clinical knowledge and skills in the care of the geriatric population,
as well as training in dementia awareness
- Excellent leadership, coaching and mentoring abilities
- Effective communication, presentation and facilitation skills, coupled with knowledge of Adult Education principles
- Strong problem solving and analytical abilities
- Well-developed conflict resolution skills, with the ability to effectively introduce and manage change
- Demonstrated project management experience; excellent organization and time management skills, with the ability multi-task and prioritize a large volume of work
- Proficiency with MS Office, email and Internet applications, as well as electronic Healthcare Record (eHCR)
- Demonstrated ability to cultivate a respectful and bias-aware team culture.
Management & Exempt Salary Grade 6
- Salary: $109,894 to $137,367 per annum
Conditions of Employment
All applicants are expected to comply with the Region of Durham's Code of Ethics and Code of Conduct Policies throughout the recruitment process. The successful candidate will be required to provide a satisfactory Police Vulnerable Sector Check dated within six (6) months from date of hire. Additionally, tuberculosis screening will be required in accordance with the FLTCAct (O.Reg. 246/22). Proof of education, qualifications and any other job bona fide requirements will also be collected. Finally, compliance with the Mask Fit Testing Program is required and will be coordinated as part of the orientation process.
