We are currently recruiting for Nurse Practitioner to provide services to MD Care Family Health Organization. While these positions are employed by Community Healthcaring Kitchener-Waterloo, the Nurse Practitioner’s day-to-day work location (across 3 locations), schedule of hours, as well as applicable policies and procedures, will be determined by MD Care Family Health Organization Team to which they are assigned.

Job Summary The Nurse Practitioner provides care within the MD Care Family Health Organization interdisciplinary team, working collaboratively with physicians and other health care professionals to support patients and families across the lifespan. The Nurse Practitioner helps improve access to care, enhance health outcomes, and support coordinated, team-based primary care for diverse patient populations
Responsibilities
1. Perform assessment, diagnosis, treatment, health education/counselling and other clinical activities in accordance with the College of Nurses of Ontario standards of practice for Registered Nurses in the Extended Class (NPs) and all relevant provincial legislation and regulations.
2. Work from a Social Determinants of Health perspective to improve health outcomes for individual clients and/or for the development of community health and well-being, and to reduce health inequities.
3. Triage client needs either over phone, or in person.

Maintain accurate and up to date clinical notes in accordance with NP’s professional standards and organizational policy.
5. Participate in the development, implementation, monitoring and evaluation of health promotion/disease prevention programs for individuals and client populations as determined by the Health Centre.
6. Work during both regular and extended hours (evenings/weekends). A minimum of one evening per week may be required.
7. Participate in team and staff meetings and other meetings and committee work as appropriate to support both service delivery and organizational goals.
8. Work individually and collaboratively in the development and implementation of quality improvement initiatives and activities.
9. Work in a manner that preserves privacy and confidentiality.
10. Work in a manner that ensures client safety and minimizes risk to clients, volunteers and the Health Centre.
11. Work in a manner that demonstrates self-reflection and personal accountability for work performance.
12. Collect, analyze, and report on data and relevant information as required.
13. Participate in research as determined by the Health Centre.
14. Adhere to all Health Centre and MD Care Family Health Organization policies.
15. Adhere to appropriate infection control.
16. Perform any other duties as assigned by your supervisor.
Qualifications
1. Completion of a recognized Primary Care Nurse Practitioner program.
2. Master’s degree in nursing; or equivalent recognized education and experience.
3. Current certification with the College of Nurses of Ontario-Extended Class and a member in good standing.

1 year minimum experience as a Primary Care NP working with all ages.
5. Comfortable counselling around a wide range of health problems common to family medicine. Some examples would include weight loss/gain, iron and vitamin deficiencies, IBS/constipation, Celiac disease, inflammatory bowel disease, eating disorders etc.
6. Valid Ontario Drivers License.
7. Knowledge of the health delivery system, community and social services and the relevant legislation related to Nurse Practitioners in Ontario.
8. Proven experience working in a cooperative team environment; and
9. Demonstrated leadership in promoting a shared responsibility for professional practice and development.
10. Experience working effectively with culturally and economically diverse
11. Ability to obtain a Police Vulnerable Sector Check satisfactory to the Health Centre.
12. 2-Factor Authentication for Outlook is App based (Microsoft Authenticator Application) and used to ensure the security of our online accounts. 2-FA requires the use of a personal mobile device with a recent operating system.
Hours: 25 hours per week
