The Athens District Family Health Team, located in beautiful Eastern Ontario between the historic Rideau Waterway and the world-famous Thousand Islands, is seeking a full time Nurse Practitioner to join our team.

Reporting to the Lead Physician and Executive Director, you will work in a highly collaborative, multidisciplinary environment alongside physicians, nurse practitioners, nurses, a health educator, and dedicated administrative staff who believe that a positive workplace culture is just as important as excellent patient care.

In this role, you will provide a full scope of primary health care services to our 4,100 patients, including preventive care, health promotion programming, and timely acute care. You will be supported by colleagues who share knowledge, step in to help, and celebrate each other’s successes because we know strong team relationships translate into better care for our community.

You will also collaborate with our trusted community partners providing laboratory, diabetes, lung health, eye health, and mental health services, ensuring patients receive seamless, well-rounded care onsite.
